Schedule And Daily Routine

To provide  the best pet care , I ask that you supply me with a typed schedule,  which I keep confidential in your file, reflecting the following:

1.Dates and times my services are requested. 

2.Your  pet’s daily routine, i.e. feeding, play, exercise, bathroom, treats,  sleeping arrangements, medications, grooming, preferences, propensities,  etc., and completed the attached pet(s) profile. 

3.Phone number where you can be reached in the event of an emergency and local emergency names and phone numbers.

 

4.If  applicable, on a separate sheet of paper please provide detailed  directions to activate and deactivate any security system. These  directions will be returned to you.
